Supported Independent Living

Certified Care Solutions Ltd provide unregulated supported living placements for young people aged 16+, caring for children with mental health and behavioural problems.

Our aim is to to provide tailored person-centred care, support and safe accommodation for young people and assist them develop and enhance their independent skills as they transition to independent adult living. We carry out a comprehensive assessment for each young person so we can determine their areas of needs and enable them to reach their goals and live fulfilling life whilst promoting their independence, diversity, choice, and respecting their privacy.

Our Supported Living service offers sole 1:1, 2:1, 3:1 occupancy for young people who require extra care and support and multiple occupancy accommodation for young people who are able to live independently with support.

Safe accommodation

Safe accommodation available 365 days a year with 24 hour support depending on the needs of the young person. All our accommodation is maintained to a high standard and furnished to meet the needs of young people.

Finance Assistance

Assistance with finances, budgeting and funding from the local authority.

Children in care

A child who has been in the care of their local authority for more than 24 hours is known as a looked after child. Looked after children are also often referred to as children in care, a term which many children and young people prefer. Certified Care Solutions Ltd are aware that children in the care are still too often threatened with financially-driven moves, or made the victims of poor planning and decision-making. We are here to give them the help, support and guidance to have the best outcomes. We help homeless teenagers and young offenders leaving secure units with nowhere to go. Certified Care Solutions works closely with the local authorises and other multi agencies to get them the support and care they need.

Bespoke health and well being routine

Build a bespoke health and well being routine, support with attending medical appointment such has dentist, doctors, opticians and leisure activities.

Support Access to Developmental needs

Support accessing education, employment, training, coaching, access to advocacy services , mentorship and counselling programs.

Care leavers

For some looked after children, leaving the care system can be a challenging time. The latest statistics published in March 2022 indicates that, the number of children looked after (CLA) by local authorities in England rose to 82,170, up 2% on last year and continuing the rise seen in recent years. Certified Care Solutions Ltd are here to cover the gap and assist the local authorities to accommodate, support and offer guidance for the care leavers.
In all nations of the UK, children leaving care at 18 are entitled to support from their local authority until they are at least 25 depending on their circumstances . England and Wales are governed by the Children (Leaving Care) Act 2000.
Certified Care Solutions Ltd assess and meet care leavers’ individual needs, develop a pathway plan and help them with the transition in preparation for them to leave care. This includes assisting them to find somewhere suitable to live.

UNACCOMPANIED CHILDREN

Unaccompanied children have often lived through years of trauma and arrive profoundly affected by what they have witnessed and are therefore amongst the most vulnerable in the country.

Such children find themselves in need of care, protection, and support as they are alone. They also need support to deal with the effects of severe trauma and mental health issues.

Many unaccompanied children face great uncertainty and are surviving in an unfamiliar environment and culture, with strange habits, rules and language.

At Certified Care Solutions Ltd we try to understand these children and provide basic levels of support, and friendship. We also undertake assessments to recognise and understand the young person’s needs.

We build a support programme, in order to determine the degree of psychological and/or emotional help the unaccompanied child requires.

Assessments will also include the child’s needs in relation to their health, disability, education, legal requirements, religious beliefs, racial origin, culture and linguistic background.

Supported Living Steps

Assessment Referral: Formal Request to access by Commissioner, Individual, Parent, Referrer.

Assessment Process Commences: Assessment Manager visits the individual and other relevant parties to gather information and compiles assessment.

Placement Offered: Email confirming capacity for placement sent to referrer/commissioner,

Placement Not Offered: Referrer informed, our involvement ends.

Placement Details Agreed: Start date / move in time confirmed. Assessment report completed. Transition planner identified.

Transition Planning Commences: Comprehensive planning process to ensure smooth Transit.

Placement Offered: Shadow shift patterns agreed, preparations completed eg. start times and length of shadow shifts.

CaP Visits Individual: Transition Planner visits Individual and other relevant people and dates and time-scales agreed.

Preliminary Support Plan: Individual support starts. Get to know surroundings and trail planned activities.

Support Planning: Detailed Support Plans Finalised and support team confirmed.

Overnight/day visits to identify support delivery location

Move In: Individual settles in to their new home.

Review: Six week review held with relevant parties to monitor progress and highlight any necessary changes.
